 1. FORMS FOR ABANDONMENT OF NON-LEAKING UNDERGROUND TANKS:

Homeowner Statement:
You must notify the the Nassau County Department of Health, in writing, seven days in advance of the abandonment or removal and use the Bureau of Environmental Protection Affirmation of Non-Leaking Tank form (PDF)

Contractor Statement: seven days in advance of the abandonment or removal to the Nassau County Department of Health.
Your contractor must submit the Small Facility/Homeowner Tank Abandonment Notification Form (PDF)

2. Forms For Removal of Aboveground and Basement Tanks

Your contractor must notify the the Nassau County Department of Health, in writing, seven days in advance of the removal and use the
Tank Abandonment/Removal Notification Form (PDF)

Please Note : original Affirmation Form must be notarized. Completed forms with appropriate fee must be mailed or delivered in person to the Bureau of Environmental Protection, Nassau County Department of Health, 106 Charles Lindbergh Blvd, Uniondale, New York, 11553.

3. SMALL TANK REMOVAL AND ABANDONMENTS – FORMS

NASSAU COUNTY ABOVEGROUND AND BASEMENT TANK REMOVAL REQUIREMENTS
The following removal procedure is for those facilities with a petroleum storage tank of a
capacity of 1,100 gallons or less formerly used for on-site heating and /or hot water.

  • Written notification of a removal must be submitted to the Department of Health detailing the
    tank size, location, and date of removal at least seven days in advance of the removal.
  • The tank and all lines, including the vent, suction, return and remote fill ports, must be emptied
    of fuel oil.
  • All liquid and residue must be removed from the tank and connecting lines. Any waste product
    removed must be disposed of in accordance with all local, state and federal regulations.
  • The direct fill, remote fill and vent lines must be cut or removed to a point below grade. The
    lines must be permanently capped or filled with an approved inert material to prevent any
    possibility of an accidental delivery after removal of the tank.
  • After the tank is cleaned and rendered vapor free, holes must be punched in the tank to make it
    unfit for the future storage of liquids. The tank can then be disposed of as scrap.
